Health Management of Traditional Chinese Medicine

1 Health
In 1978, the International Primary Health Care General Assembly issued the "Declaration of Alma-Ata", in which re-interpret the meaning of health. "Health is a state of complete physical, mental and social well-being, and not merely the absence of disease or infirmity". It extended the meaning of health from biological aspects to the biological, psychological, social aspects, which had established a new concept of modern health.

2 Sub-health

2.1 The Concept of Sub-health
Sub-health is the healthy state of low quality and experience between health and disease. This means that there are a lot of uncomfortable feeling and symptoms in body and mind, decreased vitality, decreased reactivity and adaptability, while no disease can be diagnosed definitely.

Someone sums up sub-health as "one increase and three decreases", which means increased fatigue and decreased vatility, reactivity, adaptability.

In the mid-1980s, Berkman , the scholar of former Soviet Union discovered and proposed the concept of "the third state" for the first time. Subsequently, many scholars from different countries have been proposed some similar names, such as sub-health, intermediate state, grey state, sub-clinical state, a general malaise and so on. The name of sub-health was proposed by Chinese scholars.

2.2 The Epidemiology of Sub-health
According to dozens of studies, only 5%~15% people can attain the health standards, 15% people have been diagnosed with some diseases definitely, 60%~70% people are on the sub-healthy state between health and disease that is neither diseased nor healthy.

2.3 The Classification of Sub-health

2.3.1 The Classification of Sub-health by Components of Health

(1)Sub-health in Body
Main Manifestation: fatigue for which no recognizable cause can be found or any disease can be excluded, languidness, weakness, easily catching a cold, general physical malaise, dizziness, heaviness in the head, headache, spontaneous sweating, shortness of breath, palpitation, fullness in the chest, an irregular heartbeat, loss of appetite, clear and abundant urine, lassitude in loin and legs, cold or numb limbs, aching all over the body, insomnia, dreaminess, somnolence, decreased sexual ability, disordered menstrual cycle and so on.

(2)Sub-health in Mentality
Main Manifestation: mental fatigue for which no recognizable cause can be found, blunted in responsiveness, decline in memory, scatterbrain, disturbance of emotion, disordered thinking, panic, anxiety, impatience, irritability, feeling himself/herself inferior, nervousness, indifference, solitude, recklessness, even having the suicidal attempt and so on.

(3)Sub-health in Social Adaptability
Main Manifestation: having a poor adaptability to work and study, decreased frequency in social activity, incompatible with other people, inharmonious family relationship and so on.

(4)Sub-health in Morality
Main Manifestation:having the obvious deviation which tend to injure others and ruin oneself in world outlook, views on life and values.

2.3.2 The Classification of Sub-health by Clinical Manifestation

(1)Indefinite Statements Syndrome
Main Manifestation: uncomfortable feeling in body and mind, such as fatigue, languidness, weakness, lower resistance to illness, insomnia, long term deprivation from sleep, scatterbrain, emotional instability, even can’t having the normal daily life and work. But the various indexes of clinical examinations and laboratory tests are generally within the normal margins.

(2)Sub-clinical State
This means the pre-clinical(latent period) state of some diseases, high or low threshold of laboratory tests, such as on the high margins of blood glucose, blood lipid, blood uric acid and blood pressure, or on the low margins of blood calcium, blood iron.

(3)Pathogen Carrier
The person who is a carrier of pathogen has the abnormal indexes of laboratory tests, but the normal physiological function, psychological and social adaptability, such as hepatitis B carriers, tubercle bacillus carriers and so on.

(4)In the State of High Risk Factor
Overweight, smoking, excessive tension and so on.

2.4 The Causes of Sub-health

(1)Highly Mental Stress
Because of the intense competitions in modern society, our work and study load have been becoming more and more heavy, our mind have been under the high level of stress, which will impair our health.

According to the studies, long-term mental stress can impair the health in the following aspects:
a. Triggering the acute or chronic irritable reaction, which in turn directly impair the cardiovascular system and digestive system. Then high blood pressure [hypertension], racing heart[tachycardia], hardening of the arteries [angiosclerosis],
heart attck, stress ulcer and so on will occur.
b. Triggering the mental fatigue and reducing cognitive function.

(2)Unhealthy Lifestyle

(3)Environmental Pollution
The environment pollution such as the industrial smoke and dust, emissions from automobiles, radiation, noise, light, electromagnetic wave and so on have been becoming more and more severe. These will disturb our nervous and endocrine system, reduce our immunity, which will in turn have a harmful influence on our body resistance to disease and recovery from deseases.

(4)Other Factors
Bad mental, psychological stimulation, genetic factors and so on.

2.5 The Evaluation Method of Sub-health

2.5.1 Scale Evaluation
Cornell Medical Index(CMI), Self-rated Scale by Delphi, Symptom Checklist-90 (SCL-90), Questionnaire for Survey of Constitutions in Traditional Chinese Medicine, Criteria for Classification and Diagnosis of Constitutions in Traditional Chinese Medicine.

2.5.2 Test Evaluation
a. Multifunctional Microscopy Diagnostic Instrument(MDI)
Also called one drop of blood test. The blood sample can be magnified 15, 000 times by MDI. Therefore, we can directly observe the shape and quantity of various kinds of blood visible components, such as erythrocytes, leucocytes, blood platelets, cholesterol clefts, chylomicron[microscopic particles of emulsified fat], uric acid crystals, lipid plaque, plasma urealyticum, protoplast-chip and so on. 3 Health Management
In 1996, World Health Organization (WHO) stated in "Meet the Challenges of the 21st Century" that we shouldn’t take diseases but health as our main research object in the 21st century medicine. (3)Preventing the Recurrence of Diseases

3.2.2 “Preventive Treatment of Diseases” and Chinese Culture of Health Preserving

Example 1:Bian Que and Cai Huangong
Bian Que went to see Cai Huangong, stood for a while and said, "You are sick in your skin, and will turn worse without treatment. " Huanhou said, "I'm not sick. " After Bian Que went away, Huanhou said, "The doctors like to treat people who don't have any diseases, as their own credit!"
Ten days later, Bian que went to see him again. He said, "You have a disease in your flesh, and it'll be heavier without treatment. "Huanhou didn't reply. After Bian Que went out, Huanhou showed his disatisfaction again.
Ten days later, Bian Que went to see him again. He said, "You have a disease in your stomach. It'll be even heavier without treatment. "After Bian Que went out, Huanhou was also displeased.
Another ten days later, Bian Que met Huanhou and went back immediately. Huanhou specially sent people to ask why. Bian Que replied, "The disease in the skin can be achieved by hot medicinal compress; disease in the flesh can be achieved by acupuncture;disease in the stomach can be achieved by medical decoction. While disease to the bone marrow, which is in charge of the God of life and death by the local jurisdiction, there is no way to medicine. Huanhou's disease has gone into the bone marrow now. That is why I have nothing to say. "
Five days passed, Huanhou felt hurt and sent for Bian Que. But he had left for the Kingdom of Qin. Huanhou then died.

This popular story has found its way to us through the ages. Its core content is "Preventive Treatment of Diseases".
As a matter of fact, Huangdi Neijing [The Yellow Emperor's Canon of Internal Medicine] has proposed the concept of "Preventive Treatment of Diseases", and emphasized the importance of prevention of diseases.

For example:
Plain Questions states: "Wise men always give the preventive treatment before a disease occurs and prevent a disturbance before it starts…. Taking medicine until a disease comes on or preventing a disturbance until it starts is no different from digging a well until one is thirsty or making weapons until a battle starts. That is too late. "

Example 2:Bian Que’s Art of Healing

Bian Que was a famous physician in ancient China. One day the King of Wei asked him, “You and your two brothers are all skilled in medicine, which of you is the best?
Bian Que answered, “My eldest brother is the best, the next is my second brother , and the last is me. ”
The King asked again, “Then how is it that you are the most famous?
Bian Que replied, “My eldest brother treats a disease when it is just beginning to show symptoms. What he does is to remove the pathogen--members of my family can all see that but other people cannot. That’s why he doesn’t enjoy wide recognition. My second brother cures a disease in its early stages. People thus mistake him as only capable of treating minor illnesses. So he is merely known within this locality and its neighborhood. As for me, I treat a disease when it is already well developed and very serious. People observe me perform bloodletting by injecting tubes into vessels or applying medical ointment on the skin, and thus look upon me as well versed in medicine. So I have become well known all over the country.
The King was quite satisfied and said, “You’ve given a cogent explanation. ”

Sun Simiao, the famous physician of Tang Dynasty distinguished the levels of doctors in Beiji Qianjin Yao Fang[Prescriptions for Emergencies Worth a Thousand Gold] by the following way. The high-level doctors prefer to adopt the method of health preserving and keeping fit when people are in good health. The middle-level doctors choose to nurse impaired health before a disease occur. The low-level doctors begin to treat a disease after it has happened.

3.2.4 The Methods of “Preventive Treatment of Diseases”
The previous physicians of all dynasties put the health preserving as top priority. They thought that health preserving is the fundamental way to accomplish the "Preventive Treatment of Diseases".
The basic principles are taking good care of the kidney essence, strengthening the functions of Zang-fu Organs, regulating and preserving the Yin-yang.

Plain Questions states: "Avoid Xuxie and Zeifeng[Xuxie and Zeifeng refer to all abnormal climatic changes and exogenous pathogenic factors. ]in good time. Keep the mind free from avarice. In this way, Zhenqi [Primordial Qi] in the body will be in harmony, Jingshen (Essence-Spirit) will remain inside, and diseases will have no way to occur".

"People who knew the Law, followed the principle of Yin and Yang, and were in homony with Shushu (calculations and arts), ate wisely, lived regular daily life without random labour. As a result, with the body and spirit going along, they were able to fully live the natural span of life, that is a hundred years, before death".

The above states are trying to advise people to strengthen the cultivation of mind, pay attention to the mental and psychological health, adapt themselves to the climate change, be temperate in eating and drinking, avoid partiality for a particular kind of food, get into regular habits in daily life, keep a proper balance between work and rest, do exercises, improve the physique and so on.

Sun Simiao, the famous physician of Tang Dynasty can live to 101 years old at the times that "Men who reach the age of seventy have always been a rarity". Some secrets of his longevity are "Often combing hair, often knocking teeth, often swallowing saliva", " taking a walk after a meal, stroking the hypochondrium, chest and abdomen by your hand".

4 The Constitution of Human Body and Health Management

4.1 The Theory of Constitution in Traditional Chinese Medicine

4.1.1 The Concept of Constitution
Constitution is a kind of living phenomenon that exists objectively. It is the comprehensive and relatively stable characteristics that living body show itself in the aspects of morphological structure, physiological function, personality and mentality.

Constitution presents the individual differences of Zang-Fu Organs, Qi-Blood and Yin-Yang with the differences of individual morphology, function and mentality. In physiological aspect, it presents itself with the individual differences of function, metabolism and response to external stimuli. 4.1.2 The Types of Constitutions of Human Body
After over 30 years of effort, the Chinese medical experts have put forward nine kinds of basic constitutional types of human being, which are Healthy Type, Qi-Deficiency Type, Yang-Deficiency Type, Yin-Deficiency Type, Phlegm-Dampness Type, Dampness-Heat Type, Blood-Stasis Type, Qi-Stagnation Type, Allergy Type. They also have designed the Questionnaire for Survey of Constitutions in Traditional Chinese Medicine, and Criteria for Classification and Diagnosis of Constitutions in Traditional Chinese Medicine, which is a standard tool for the classification of constitutions.
Of course, people who are single constitutional type are often rare in real life, compound constitutional type are the majority.

4.1.3 The Characteristics of Constitution
The formation of constitution is the result of both prenatal heredity and postnatal acquisition. The former is a decisive factor, which makes the constitution possessing relative stability and predictability. The latter means that constitution can be altered by other factors, which makes the constitution possessing alterability and adjustability.

4.2 The Constitution and Occurrence of Subhealth
Healthy Constitution means the healthy state, which only accounts for 32. 75 percent of population. The other eight types of deviated constitutions mean that the balance between Zang-Fu Organs, Qi-Blood and Yin-Yang have been disturbed, and haven’t yet became diseases. These belong to the sub-healthy state between health and diseases, which account for 67. 25 percent of population.

Annex: The relationship between various deviated constitutions and sub-health
a. Qi-Deficiency Type, Dampness-Heat Type, Qi-Stagnation Type: The vitality of body has been reduced. The metabolic wastes can not been discharged from the body timely. The person with these types of constitutions has been depressed. Therefore, it is easy to turn into Sub-health in Body[Fatigue Type]
b. Yang-Deficiency Type, Dampness-Heat Type, Blood-Stasis Type: The pain will occur because the poor blood circulation can not nourish the body sufficiently. d. Qi-Deficiency Type, Yin-Deficiency Type, Qi-Stagnation Type: The physical ability has declined. The emotion has been in flux dramatically, which sometimes present easily anger or depression. Therefore, it is easy to turn into Sub-health in Mentality.
